學生請假說明(學生Q&A)

 

壹、提醒事項

I. Notes:

一、106年12月22日106學年度第1學期學生事務會議修訂通過「學生請假辦法」自本(107)學年度第1學期起施行。本辦法公告於「生活輔導組>法規」,網址:http://www.cc.ncku.edu.tw/rule/content.php?sn=248

1. The National Cheng Kung University Regulations for Student Leave of Absence, revised and approved by the Student Affairs Council in the first semester of the 2017-18 academic year on December 22, 2017 shall take effect from the beginning of the first semester of the 2018-19 academic year. These regulations are publicized on the webpage of the Student Assistance Division at http://www.cc.ncku.edu.tw/rule/content.php?sn=248 .

The Student Leave Application Online System is available at http://i.ncku.edu.tw/ .

二、學生請假應登入「學生請假系統」提出申請,並依相關規定辦理,除不可抗力因素外,不再受理紙本請假單之申請。

2. Students must log into the Student Leave Application Online System to apply for leave according to relevant regulations. Written applications will not be processed except unforeseen contingencies beyond the control of the applicant.

三、除病假外,學生請假事先提出申請。重病或臨時發生緊急、重大不可抗拒之變故,應於請假日最後一天次日起算五天內(含假日),至「學生請假系統」提出申請。

3. Applications for leave of absence, except the application for sick leave, should be submitted for approval in advance of the leave. In case of severe illness, emergency, or unforeseen contingencies, students will be allowed to complete an application for leave on the Student Leave Application Online System within 5 days from the last day of the leave (holidays included).

四、學期中寒、假期間:

  1.非公假:因參與校外學習活動不在校內,同時活動期間並無任何課程,無須登入學生請假系統提出申請。

  2.公假:因公參與各項活動期間(平日或假日)如無任何課程,仍須登入學生請假系統提出申請。

  3.為完善您個人的學習紀錄,請逕至 「數位學習歷程檔e-Portfolio >登錄校外學習紀錄」,網址 : http://eportfolio.ncku.edu.tw/index2/index.php 。

 However, they are advised to go to the website of e-Portfolio at http://eportfolio.ncku.edu.tw/index2/index.php to update your record of learning activities.

五、「學生請假系統」提供本校全體師生請假程序之便利性,有關「准假權責」之准假程序,請同學務必與師長保持聯繫,並且留意審核時程,以避免逾期未完成請假程序而受到退件

5. The Student Leave Application Online System is established as a student leave application platform convenient to be operated by NCKU faculty members and students. Applicants must keep their instructors and the offices concerned of approval procedures to complete their applications within the designated deadline.

六、請本校全體師生、系所院及行政單位主管、同仁,於登入「學生請假系統」申請假單、審核假單時,可於「相關說明文件」項下,詳閱「假單簽核流程說明-學生請假流程圖」「學生請假辦法」「公假說明」「非公假說明」等,以利學生請假、師長及單位主管准假之流程順利,共同維護學生權益。

6. All NCKU members who log into the Student Leave Application Online System to process student leave applications are advised to read “Related Instructions” including Student Leave Application Flowchart, National Cheng Kung University Regulations for Student Leave of Absence, Instructions for Official Leave, and Instructions for non-official Leave, in order to facilitate their operation and secure the rights and interests of students.

貳、Q&A 學生篇

II. Q&A (Students)

Q1:學生需要請假時,應如何提出申請?如何選擇請假假別?

How do students submit a leave of absence application?

How do applicants select the type of leave on the system?

A:

(一)每一位需要請假之學生,應登入「學生請假系統」提出申請。

(二)登入後,於「假單申請」選擇請假類別,如下(點選請假類別後顯示假單申請說明,請詳閱):

   (1)「非公假申請」:含病假、事假、喪假、 產假(含產前、分娩、哺育及陪產假)、生理假、器官捐贈假(含骨髓、器官)及學期考試假。

   (2)「公假申請」:依據學生請假辦法規定之公假事由提出。

 A: 1. Students must log into the Student Leave Application Online System to apply for a leave of absence.

2. Applicants select the type of leave under the link of Leave Application as follows (please read the Instructions for Leave Application):

 (1) Non-official leave: including sick leave, personal leave, bereavement leave, maternity leave (including prenatal care, postnatal care, baby nurturing and paternity leave), menstrual leave, and leave for organ donation (bone marrow or organ), and leave from term examinations.

  (2) Official leave: applications submitted according to the NCKU Regulations for Student Leave of Absence.

Q2有哪些請假假別或請假天數,須檢具相關證明文件?如何繳交?

In terms of type of leave or duration of leave, are applicants required to submit a certificate or supporting document?

How do applicants submit a certificate or a supporting document?

A:

(一)假別:公假、喪假、產假(含產前、分娩、哺育及陪產假)、器官捐贈假(含骨髓、器官)。

(二)天數請假天數3天(含)以上。

(三)證明文件繳交:應以「電腦上傳」附件,可上傳圖片PDF檔,再點選「送出請假單」。

1. Types of leave: Official leave, bereavement leave, maternity leave (including prenatal care, postnatal care, baby nurturing and paternity leave), menstrual leave, and leave for organ donation (bone marrow or organ)

2. Duration of leave: More than three days.

3. Applicants shall upload a certificate or supporting document as an attachment in the format of image or PDF before they click on “Submit” to complete submission of the leave application.

Q3:請假單之請假程序如何進行?

How do students submit a leave of absence application online?

A:

(一)「非公假申請」:請選擇「請假假別」,依序填寫各欄位請假資料,最後點選「送出請假單」。

(二)「公假申請」:請依核准公假權責選擇「權責單位」,點選承辦老師、系主任(所長或學位學程主任);或行政承辦人員、主管,再依序填寫各欄位請假資料,最後點選「送出請假單」。

1. Non-official leave: Please select “Type of Leave”, then fill out the designated columns, and click on “Submit” to complete submission of the application.

2. Official leave: Please select “Unit of Authority”, click on the designated approval authority (responsible instructors, department chair, institute or program director, responsible staffer or chief of the administrative unit, and click on Submit to complete submission of the leave application.

Q4:學生請假單提出申請後,可否查詢假單審核進度?可否删除請假單?

Are students allowed to keep track of their leave application approval status?

Are applicants allowed to delete/cancel the leave application?

A:

(一)可以。於「假單查詢」項下,皆可查閱每一筆請假單「審核狀態」之明細。

(二)請假單申請錯誤時:於「假單查詢」項下:點選該筆請假單號>删除>儲存(删除成功)>確定。

1. Yes. They are allowed to check out the details of Approval Status under the link of Application Inquiry.

2. In the event of an incorrect application, applicants are allowed to delete/cancel it under the link of Application Inquiry as follows:Select Serial No.>Delete/Cancel> Save >Confirmed(Completed)

Q5如因故無法事前請假或逾期請假時,應如何辦理?

If applicants fail to submit an application for approval in advance of the leave, or if applicants submit an overdue application, what should they do to complete the leave application?  

A:

(一)依學生請假辦法第三條第二、三項規定:「…(前略)。但應先以電話、書信或委託他人等方式向授課老師報備;若因故無法與授課老師取得聯絡者,亦得向導師或系主任(所長或學位學程主任)報備。」

(二)如因:(1)急病。(2)臨時接獲親屬喪故通知。(3)臨時發生緊急、重大或不可抗拒之變故。致逾期請

假時,應於請假日期最後一天次日起算五天內(含假日),於「學生請假系統」提出申請。

(三)如果已逾上項規定之期限後才提出假單申請者,應填寫「詳細逾期理由」,並依「准假權責」規定之准假程序辦理請假單。

EX(1)○○○同學107/8/25因身體不適須請病假,應於請假系統提出申請。

無逾期:實際登入系統提出請假申請日期為8/30,此請假單計算期間:自8/26~30止,仍為規定期限內。

EX(2)同上範例

已逾期:實際登入系統提出請假申請日期為9/2,此請假單計算期間:8/26~9/2止,已逾規定期限,須填寫逾期申請理由

1. Applicants are required to inform their instructors in advance by phone, by mail or by proxy. Should they fail to contact their instructors, they are required to report the leave to their advisors or department chair (director of the graduate institute or the program), as is stipulated in the second and third paragraphs of Article Three of the NCKU Regulations for Student Leave of Absence.

2. In case of (1) acute or sudden illness, (2)a short notice of obituary, and (3) emergency or unforeseen contingencies, applicants are allowed to complete an application for leave on the Student Leave Application Online System within 5 days from the last day of the leave.

3. Those who submit a written application after the designated deadline set in (2) should specify reasons for an overdue application, which shall be processed according to the rule of approval authority.

 Ex.: Student A taking a leave for illness on August 25, 2018 should submit a leave application on the Student Leave Application Online System.

(1) Not overdue:

Student A submitted an online application on August 30 within the designated period from August 26 to August 30.

(2) Overdue:

Student A submitted an online application on September 2 after the designated deadline of August 30. Student A must specify reasons for an overdue application.

Q6請假系統送出請假單後,是否已完成請假程序?應注意哪些事項?

If I have submitted an application on the Student Leave Application Online System, does it mean that I have completed my application? What should I do for a failed submission?

A

(一)

(二)假單申請無法送出之應注意事項:

 (1)系統顯示請假單有資料欄位未填。

 (2)逾請假辦法規定應申請期限後提出請假者,應填「詳細逾期理由」,未填寫者。

 (3)准假權責之師長、單位主管可退件(應備理由);備查單位或系統自動退件(退件理由),如:

A.請假理由不符或不詳者。

B.應檢具證明文件,有缺、漏者。

C.請假事由與選擇之假別不符合等情形。

   D.核假之師長或單位主管不同意准假。

E.請假單送出後,於請假申請日次日起算20天內完成准假程序者,系統自動退件

(請同學務必與師長保持聯繫並留意審核時程,以避免逾期未完成請假程序而退件)

1. No. You have not completed your application.

2. Notes for a failed submission:

  (1) The system indicates some columns required to be completed.

  (2) The applicant fails to specify reasons for an overdue application.

  (3) Applications may be rejected by instructors or unit chiefs, or automatically rejected by the unit for approval ratification or the system for reasons as follows:

   A. Applications with inconsistent or unspecified reasons

   B. Submission of certificates is required.

   C. Reasons are inconsistent with the type of leave.

   D. The application is disapproved by instructors or unit chiefs.

   E. Upon submission, an application that fails to be approved within the following 20 days will be automatically rejected online.

Applicants should keep their course instructors and the offices concerned informed of the approval procedures to avoid application rejection. In the event of a rejected application, applicants shall submit a new application online if they are required to do so.

Q7:學生如何得知請假單因為上項原因被退件?退件後仍需補請假,可否重新提出

申請?

How do applicants know why their applications have been rejected? In the event of a rejected application, can applicants submit a new application online?

A:

(一)學生於系統請假時,應填寫個人正確、清楚之email,如有請假單被退件時,系統將email通知學生個人收件信箱,以利查閱。

(二)可以。被退件後如仍需補請假之學生,可於系統中「修正」被退回之假單重新提出申請。

EX○○○同學107/8/10因事提出事假申請,當日有課程計3科目須請假,依請假辦法規定

內之准假權責:科授課老師、導師。

(1)無退件:學生提出請假>系統分送並通知每一科授課老師有假單待審核>每科授課老師皆准假>通知導師待審核>經導師於8/30前准假>由生輔組備查>系統email通知學生請假單已完成備查。

(2)有退件:

A、如上列准假流程>授課老師及導師8/30未於系統完成准假程序>系統於8/31予以退件email通知請假學生>該筆請假單為無效假單。

B、該筆請假單遇系統退件後,學生仍需補請假者,應至學生請假系統提出申請。

1. Students applying for leave online must enter their email address. The system will inform the applicant of a rejected application by email.

2. Yes. Applicants are allowed to submit a new application online if they are required to do so.

Ex.: Student A submitted an application for a one-day personal leave from three courses scheduled on August 10, 2018. According to the regulations, a less-than-three-day leave application shall be approved by course instructors and the advisor.

(1) Application completed:

Procedures: an application submitted >instructors informed by the system of an application pending approval> the application approved by all instructors> the advisor informed of an application pending approval> the application approved by the advisor by August 30 >Student Assistance Division for verification> Student A informed by email of the application completed.

 (2) Application rejected:

  A. In the approval procedures> Course instructors or the advisor failed to approve the application by August 30. >The application was rejected by the system on August 31 while Student A was informed of the rejection by email. >The application has been invalid.

B. In the event of a rejected application, applicants are allowed to submit a new application online if they are required to do so.

Q8:學生請假日,遇「准假權責」審核假單之師長或單位主管新、舊更替,致准假程序(請假進度)無法完成時,應如何處理?

If my application fails to pass through the stage of approval procedures online due to a replacement of the department chair (graduate institute or program director), or chief administrator of the responsible office, what should I do to complete the application?

A:

(一)「非公假申請」

 (1)請假日每學期初(如:當學期未開學之前或開學第一周),授課老師名單尚未確認時,應於第二周以後再提出請假申請。

 (2)如因此致請假日已逾規定申請期限,仍應至請假系統提出申請,可敍明逾期理由:「授課老師名單尚未確定」

 (3)准假程序如遇系主任(所長或學位學程主任)更替無法完成者,請洽生活輔導組辦理(06)2757575#50340

(二)「公假申請」如遇系主任(所長或學位學程主任),或因公派遣學生出任公務之行政單位主管更替,以致准假程序無法完成時,請學生自行取消(删除)該筆假單,並重新提出申請。

1. Non-official leave:

(1) If the roster of course instructors is not finalized before the start of the semester or in the first week of classes, you should submit their applications from the second week.

(2) In the event of an overdue application under this condition, you should specify “The roster of course instructors is not finalized” as the reason in the designated form on the system.

(3) An applicant whose application fails to be processed through the stage of approval procedures online due to a replacement of the department chair(graduate institute or program director) shall contact the Student Assistance Division at (06)2757575#50340 to process an application.

2. Official leave:

You should cancel your application that fails to be processed through the stage of approval procedures online due to a replacement of the department chair (graduate institute or program director), or chief administrator of the responsible office before you are allowed to submit a new application.

Q9:學生請假事由,如為校外活動之情事,是否應至學生事務處軍訓室,完成「校外活動登錄程序」

In case of a leave application for extramural activities, do I need to complete registration for off-campus activities at the Military Training Division under the Office of Student Affairs?

A:

(一)。請假事由如為校外活動之情事,請於假單送出後,至軍訓室網頁>表單下載>「校外活動登錄表」填寫印出後送交軍訓室。

(二)如為團體活動,請學生務必轉知領隊(隊長或帶隊師長)代表填寫一份即可,送交軍訓室完成登錄。

1. Yes. Upon submission of your leave application for an extramural activity, please complete the National Cheng Kung University Registration Form for Extramural Activities at http://b3c008.web3.ncku.edu.tw/p/412-1055-5465.php?Lang=zh-tw, which shall be printed out and submitted to the Military Training Division.

2. In the event of a team/group activity, please inform your leader (captain or instructor) to complete and submit the form on behalf of all applicants.

Q10:如請假系統無法登入申請時,是否可以改用紙本請假單提出申請?

If I fail to log into the leave application online system, can I submit a written application?

A:

(一)否。原則上,學生請假應登入請假系統申請,不受理紙本請假單之申請。

(二)如為下列因素致無法登入請假系統申請時,可改用紙本請假單依准假程序申請:

(1)無課程代碼。(2)暑修(無課程代碼)。(3)任何不可抗力因素致無法線上申請之情形。

1. No. In principle, students should log into the system to apply for a leave of absence.

2. No written application will be processed except under special circumstances as follows:

     (1) Courses without designated codes, (2) Courses offered for a summer session without designated codes, and (3) Failure to submit an online application due to unforeseen contingencies.

Q11:何謂「學期考試」假?應何時提出申請?

How is a leave from “term examinations” defined? When should a student apply for a leave from term examinations?

A:

(一)定義:依本校學則第七章第十八條第三款「學期考試:於每學期結束時在規定時間內舉行之。」。

(二)依學則第七章第廿一條:

(1)第一項:「學生因重大事故,不能參加學期考試,需補考者,應依學生請假辦法規定,辦理請假手續,請假期間所有之應考科目均列入請假。(後略…)。」

(2)再依第二項:「期末考試除因公、病、生產、哺育三歲以下子女或配偶、二親等以內之親屬喪故而請假…(後略…)。」

(三)據上規定,學生因故未能參與學期末考試時,應登入「學生請假系統」提出請假申請。

1. Definition: Term examinations refer to final examinations scheduled for the final week of the semester, as is stipulated in Subparagraph 3 of Article 18 under Chapter 7 of the NCKU Academic Regulations.

2. As Article 21 under Chapter 7 of the NCKU Academic Regulations states:

       (1) In Paragraph 1: Students who cannot attend regular midterm/term examinations due to a major incident, and thus need to complete make-up examinations afterwards, shall apply for a leave of absence from regular examinations according to the NCKU Regulations for Student Leave of Absence. All examinations scheduled for the dates during the leave of absence shall be specifiedon the application form.

 (2) In Paragraph 2: Those taking leave of absence from regular end-of-term examinations due to official duty, illness, pregnancy, care for their children aged below three or a spouse, or the funeral of a relative by the first or second degree of kinship,...

3. According to the rules stated above, students who fail to attend term examinations shall apply for a leave of absence on the Student Leave Application Online System.
